Who qualifies
You may qualify if you are a New York State Medicaid member and at least one of the following applies to you or someone in your household.
Required
You must be a New York State Medicaid member.
A valid Medicaid ID (2 letters · 5 digits · 1 letter) is required to apply.
Plus at least one of the following:

You are pregnant or recently had a baby
- 02
You have a chronic health condition
- 03
You have a disability
- 04
You need help with food, housing, or transportation
- 05
You have a serious mental illness or substance use disorder
- 06
You have a child under 18 with a chronic condition
