NourishNY LLC is a New Jersey limited liability company that helps New York Medicaid members apply for the NYS 1115 Waiver Nutrition Benefit through its Social Care Network ('SCN') partner. NourishNY is not a hospital, health plan, or government agency. The Nutrition Benefit itself is a New York State Medicaid program; eligibility decisions are made by the State and our SCN partner, not by NourishNY.

To apply through NourishNY, you must be a New York State resident and an active New York Medicaid member, and at least one of the eligibility categories described on the Who Qualifies page must apply to you or a member of your household. Submitting an application does not guarantee enrollment; eligibility is verified by our SCN partner with NY State.

The information on this website is for general informational purposes. NourishNY does not provide medical, legal, or insurance advice. For medical questions, contact your healthcare provider; for benefits questions outside the Nutrition Benefit, contact NY State of Health or your local Department of Social Services.

You agree to provide accurate, current, and complete information in your application. Submitting false information may result in your application being denied, your enrollment being terminated, and may have legal consequences under state and federal Medicaid rules.

You agree not to:

• Submit applications on behalf of others without their authorization
• Misrepresent your identity, eligibility, or household composition
• Use the website to harass, defraud, or harm others
• Attempt to circumvent security measures or gain unauthorized access
• Use automated tools (bots, scrapers) without our written consent

We aim to keep the website available 24/7 but do not guarantee uninterrupted service. We may modify or discontinue features at any time. The Nutrition Benefit itself is subject to NY State Medicaid program rules and may change as the 1115 Waiver evolves.

The website and our services are provided "as is" without warranties of any kind, express or implied. To the fullest extent permitted by law, NourishNY disclaims all warranties of merchantability, fitness for a particular purpose, and non-infringement.

To the fullest extent permitted by law, NourishNY shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, arising out of or related to your use of the website or services.

These Terms are governed by the laws of the State of New Jersey, without regard to conflict-of-law principles. Any disputes shall be resolved in the state or federal courts located in New Jersey.
